Resection of an endobronchial hamartoma by cryotherapy

چکیده
A 72-year-old man was referred to our hospital with an endobronchial mass spotted on chest computed tomography (CT). On examination, he was afebrile, but complained of cough and sputum. Laboratory test results were normal. His chest CT showed an ovoid fat attenuation nodule in the left upper lobe bronchus (Fig. 1A). Flexible bronchoscopy was performed to investigate the endobronchial lesion, and a polypoid tumor was found in the left upper lobe bronchus that was leading to total obstruction (Fig. 1B). Because left upper bronchus obstruction could lead to recurrent postobstructive pneumonia, cryotherapy was performed. The tip of the cryoprobe was placed in direct contact with the tumor and was pulled and withdrawn with the frozen tissue using a bronchoscope (Fig. 2A-2C). The mass was successfully removed after repeated procedures. Histopathological examination showed mature cartilage and adipose tissue in the deeper layer, and the patient was diagnosed with an endobronchial hamartoma (Fig. 2D). At 1-month follow-up, a bronchoscopic examination showed no remnants of the mass and no tumor recurrence. Hamartomas can occur in any region of the lung, but are rarely identified as endobronchial. A majority of patients with parenchymal hamartoma are asymptomatic; the possibility of endobronchial hamartomas as a cause of postobstructive pneumonia should be considered. In a Korean study that reviewed 24 cases of endobronchial hamartoma, 10 (42%) showed Resection of an endobronchial hamartoma by cryotherapy

Two Cases of Diagnosis and Removal of Endobronchial Hamartoma by Cryotherapy via Flexible Bronchoscopy
Although endobronchial hamartoma is a rare benign tumor, most patients with endobronchial hamartoma have respiratory symptoms such as obstructive pneumonia, hemoptysis, cough, or dyspnea due to bronchial obstruction. It can cause irreversible post-obstructive pulmonary destruction, thus early diagnosis and treatment is very important. Recently, there have been cases of neodymium-doped yttrium a...
متن کاملAn Unexpected Cause of Hemoptysis: Endobronchial Lipomatous Hamartoma
Hamartomas are the most common benign tumors of the lung. Endobnronchial hamartomas are even rarer and infrequently causes hemoptysis. We report a case of endobronchial hamartoma that was originating from a segment bronchus and invisible in chest X-ray. A 63-year-old man was admitted to hospital with hemoptysis. A CT scan revealed endobronchial mass obstructing anterior bronchus of the right lo...
متن کاملEndobronchial Lipomatous Hamartoma: An Incidental Finding in a Patient with Atrial Fibrillation—A Case Report
Introduction. Lung hamartomas are the most common benign tumors of the lung. Typically, they are located in the peripheral lung, while an endobronchial localisation is rare. Case Presentation. We present a case with the rare diagnosis of an endobronchial hamartoma as incidental finding in a 69-year-old male, caucasian patient with atrial fibrillation. At first admission, the patient's exertiona...
متن کاملEndoscopic Cryosurgical Resection of Pulmonary Hamartoma with Flexible Bronchoscopy
Pulmonary hamartoma is one of the most common benign lung tumors. Well-known conventional methods of treatment for lung hamartomas include VATS enucleation or wedge resection, bronchoplasty, and others. Here we present a case of endobronchial hamartoma that was successfully treated with cryosurgery by flexible bronchoscopy.

Less than 1% of lung neoplasms are represented by benign tumors. Among these, hamartomas are the most common with an incidence between 0.025% and 0.32%. In relation to the localization, hamartomas are divided into intraparenchymal and endobronchial.Clinical manifestation of an endobronchial hamartoma (EH) results from tracheobronchial obstruction or bleeding. Usually, EH localizes in large diam...
